乙醇浸漬時間對捐贈鞏膜之影響-組織形態變化之觀察

Effects of Ethyl Alchol as a Preservative on Donor Sclera-Morphological Observations

5個以95%乙醇保存了1個月到9個月不等時間之斯里蘭卡鞏膜,以及1位因車禍死亡之國內捐贈者之新鮮鞏膜,分別接受Hematoxy lin/Eosin染色及Mason Trichrome染色之光學顯微鏡檢查,以及穿透式電子顯微鏡檢查,以了解95%乙醇濜漬保存時間長短對捐贈者鞏膜之影響。結果發現,膠原變性斑塊,膠原纖維鬆脫等現象普遍地存在於乙醇濜漬過之鞏膜光學鏡檢切片之中,而其程度則與濜漬時間沒有關聯。在電子鏡檢中,膠原纖維640A°週期橫紋模糊或消失之現象亦普遍存在斯里蘭卡鞏膜之中,其與濜漬時間長短亦與明顯之關聯。鞏膜變性的產生,依本篇之觀察,可能是發生在摘取眼球之後與95%乙醇濜漬之前,一旦浸入95%乙醇保存之後,組織形態上的變化便不再明顯進行。因此,我們以為,以95%乙醇來長期保存捐贈者鞏膜,至少在形態學變化的標準下應是可行的。

Five 95% alcohol preserved Sri-Lanca donar scleras of different preservation duration were obtained from our oculoplastic scleral reservoir to be investigated of the effects of different alcohol preservation duration on the morphological changes of the donar scleras. A domestic donar sclera, which was treated with fixatives immediately after enucleation, served as a control. Traditional Hematoxylin/Eosin stain, Masson Trichrome stain light microscopy and transmission electromicroscopy were used for morphological studies. In the H/E stain light microscopic observations, there were in general, increased spaces and enlarged spaces between collagen fibers. The Masson trichrome stain showed variable stainability in one field, and there were non-stainable plaque of variable size and shape in preserved scleras as compared to the freshly fixed domestic one. Interestingly there is no apparent correlation between the preservation duration and light microscopic shanges, although there are obvious differences between preserved scleras and control one. In electromicroscopic study the preserved donar scleras showed difinitively damaged morphology, as compared to control one, such as disappearance of the characteristic 640Å peridiocity of the collagen fibril in some areas, blurring of the 640Å peridiocity in other areas, loss of electron-dense filaments on cross section of the collagen fibril and blurred collagen fibril margins. These differences also existed between the 5 months preserved sclera and the 9 months preserved sclera although they differed in a lesser extend. In summary, the morphological changes between domestic sclera and-the Sri-Lanca ones are obvious, but the changes that can be attributed to different preservation durations among Sri-Lanca scleras are equivocal. There is probably little adverse effect from 95% alcohol preservation at least on morphology, and the maximal preservable period would be at least 9 month drawn from present observations. The morphological effects derived from a few factors, such as MacMarey-Kaufman medium preservation, age and axial lengths of the eye ball, should be further cleared out before making a more definitive conclusion.
