玻璃體基底部撕離引起之網膜剝離－病例報告
Retinal Detachment Due to Avulsion of the Vitreous Bas-A Case Report
摘要
本篇報告一例眼球挫傷後，玻璃體基底部撕離造成鼻上側巨大裂孔，繼發網膜剝離波及黃斑。因視覺模糊、視野缺失求診。眼底鏡檢查發現有玻璃體基底部撕離，因為此為外傷性網膜剝離之證病表徵因此而證實病人一年前有外傷之病史。經鞏膜扣壓術網膜復位良好視力恢復卻不盡理想。僅就此病例略述玻璃體基底部撕離的機轉、臨床表現、治療及預後。

We report a case of retinal detachment due to avulsion of the vitreous base at upper nasal quadrant with an underlying giant tear-a dialysis. Definite trauma history was obtained as we notice the pathognomonic sign of traumatic retinal detachment-avulsion of the vitreous base. We have generally discussed the mechanism, clinical pictures, treatment and prognosis of this case. Successful attachment of retina was noted during a period of 3-month follow-up after Scleral buckling. However, the visual outcome is not satisfactory due to macular involvement. In conclusion, early detection of upper nasal dialysis due to avulsion of the vitreous base with or without retinal detachment is one of the major determinant of visual outcome, before macular involvement.
