以同侧视半盲视野缺損表现之腦下垂體腫瘤－病例报告
Homonymous Hemianopsia Manifested in the Pituitary tumor-case Report
摘要
腦下垂體腫瘤所造成的视野缺捐多為單侧或雙侧之顳侧视半盲。我们在此介绍一病例，以同侧视半盲视野缺捐表现之腦下垂體腫瘤。該三十三歲女性患者除了在视野方面的表微外，無論是臨床表现，内科之生化标豁数據，頭部X光及頭部之電腦層掃描，均顯現出為一典型之腦下垂體腫瘤。於外科之開頭手術中，可见一巨大腦下垂體腫瘤延伸至右侧霸葉；追就可解释本病例在视野方面所表现之特異性。

Pituitary tumor usually induces bitemporal hemianopsia. We present a female patient wills clinical manifestation of pituitary tumor but visual field reveals homonymous hemianopsia. Skull X-ray of the patient showed ballooning of sella turcica wills lesion in dorsum sella and brain CT disclosed a big tumor occupying the pituitary fossa with extension into right parasella area. The patient recieved craniotomy with partial excision of the tumor mass, and use pathology proved the growth hormone releasing chromopltobe adenoma. The etiology of this different visual field defect is due to the lesion expanding posteriorly and compressing fibers of the optic tract, which correspond to the demonstration of brain CT and the intra Operative finding.
