單次視神經炎後視力恢復不良的臨床觀察

摘要

從臨床經驗及一些文獻的回顧，一般都認為視神經炎除非多次重覆發作，對於單次發作的病例其視力恢復都可以相當不錯。本文報告五例單次視神經炎後視力恢復不良的病例，男一例女四例，平均年齡27.6歲，其臨床特徵為典型視神經炎症狀，包括中心視力減低且常伴有動眼時疼痛，病眼瞳孔有RAPD，色覺異常，不正常VEP及視野缺損，眼底視神經盤蒼白等，雖經藥物治療，中心視力追蹤至少六個月後，視力仍是小於20/200。診斷上應先排除缺血性視神經病變，梅毒性視神經炎，營養缺乏，毒性視神經病變，惡性視神經膠細胞瘤及其它壓迫性視神經病變的可能性。

The visual recovery of idiopathic optic neuritis is always rather well except those with recurrent attacks. We present and analyze the clinical characteristics of five cases, one male and four females, age 2o to 36. They had just single optic neuritis episode which resulted in poor visual recovery, all less than 20/200, for at least six months follow-up at our neuroophthalmologic clinic. The examinations including CT, MRI and CSF study excluded the possibilities of multiple sclerosis or other inflammatory diseases.
