弧形角膜切開術及直線橫向角膜切開術後角膜表面圖譜及角膜弧度變化之研究

Keratometric and Topographic Changes after Arcuate and Linear Transverse Keratotomies in Rabbits

摘要


正常角膜或角膜移植術後之散光問題一直是臨床上的重要課題,以往解決的方法可分為眼鏡和手術方式兩種,但是如果散光度數太高則唯有手術才能矯正。本研究以兔子為對象,比較臨床上常用的直線橫向和弧形角膜切開術兩種手術方法對角膜散光和角膜表面圖譜所產生的影響。18隻兔子共分為叁組,每組六隻,左右眼分別施以單一或上下各一之直線橫向和弧形角膜切開術於6或8釐米之同心圓上,衛前和術後一、二、六星期、三個月測角膜弧度,並以角膜表面圖譜儀測量角膜表面圖譜、角膜表面規指數、和不對稱指數。結果顯示對於6釐米同心圓之單一或上下各一直線橫向比弧形角膜切開術對散光之產生效果較大。對於角膜不對稱指數,則在6釐米同心圓上下各一之弧形角膜切開術要比直線橫向角膜切開術小,其餘則沒有差別。本研究結果可提供臨床上選擇角膜切開術方法和同心圓大小選擇的參考。

To compare the astigmatism creating effect and topographic changes after arcuate and straight transverse incisions in rabbit eyes. Eighteen New Zealand white rabbits were equally divided into 3 groups. Group 1 received single ninety degrees arcuate incision around 6 mm optic zone on one eye and 6 mm straight transverse incision tangential to 6 mm optic zone on the other eye respectively. Group 2 underwent paired arcuate incisions around 6 mm optic zone and 6 mm paired straight linear incisions respectively. Group 3 received paired arcuate incisions around 8 mm optic zone and 8 mm paled straight incisions respectively. Keratometric readings and topographic changes were measured preoperatively and 1, 2, 6 weeks and 3 months postoperatively. Astigmatic changes were analyzed by vector analysis. Single and paired straight incisions have more astigmatism creating effects than arcuate incisions around 6 mm optic zone (p<0.05). But there was no difference in paired 8 mm optic zone incisions. Surface asymmetric index (SAT) was less in paired arcuate incisions than paired straight incisions in 6 mm optic zone (P<0.05), but no difference in single 6 mm and paired 8 mm optic zone incision. There is no difference in surface regularity index (SRI) among each group. Straight transverse incision (single and paired) had more astigmatism creating effect than arcuate incision in 6 mm optic zone. Paired arcuate incisions had more benefits in central symmetricity than paired angential incisions.
