Lid Splitting with Anterior Lamella Resection for Trichiasis

Trichiasis results from lid inflammation, trauma, surgery, or idiopathic cause. Methods of treatment include epilation, electrolysis, cryotherapy, argon laser ablation or wedge resection. However, we introduce a simple and effective treatment for trichiasis. We split the lid margin at the gray line with resection of the lash-bearing anterior lamella. Advance the remaining skin-muscle flap to cover all but the distal 2 to 3 mm of the anterior tarsal surface. Seventeen patients with mild to severe trichiasis were repaired by this technique between September 1990 and May 1995. Follow-up ranged from 3 months to 5 years. Of the 17 patients, 16 had excellent results as defined by a complete resolution of symptoms and no regrowth of lashes. Only one patient had a partial regrowth of lashes. The cosmetic results were good and no patient was displeased with the appearance.
