目的:溢淚是眼科門診常見之疾病,其中向於淚點狹窄所引起者也不在少數。我們用二氧化碳雷射來治療向於淚點狹窄所引起的溢淚。 方法:我們共收集主訴溢淚且有明顯之淚點狹窄息者18位33隻眼,男性6位,女性12位,年齡分佈從43歲到68歲;利用二氧化碳雷射氣化含水組織之特性行淚點整形術。 結果:在患者的主觀上和臨床觀察上,我們可將結果分為四組:第一組在術後即不再有溢淚現象者,有15隻眼(15/33);第二組溢淚明顯的改善,且能滿意於結果者15隻眼(15/33);第三組為溢淚減少但不滿意者有3眼(3/33);第四組為毫無改善者,則為”0”眼;第三組之患者我們再行一次之雷射整形後,皆能滿意於此種手術之治療。 結論:本篇是利用二氧化碳雷射在手術顯微鏡下治療因淚點阻塞而造成的溢淚,在治療後18位患者33隻眼在經過半年以上的追蹤並無不良之副作用,且所有的患者皆能滿意於結果。目前還未曾有相同之治療方式報告過。
Epiphora caused by punctal stenosis is common. CO2 laser evaporates the tissue with excellent hemostasis and less scar. Laser punctoplasty will avoid the damage of punctum and canalculus by mechanical punctoplasty. Thirty-three eyes from 18 patients with epiphora were collected. Punctal stenosis was noted. Nasolacrimal duct was patent. Under local anesthesia, CO2 laser punctoplasty was performed around the margin of punctum with 5 spots with energy about 5 watts per pulse. Follow up for 6 months, the results showed group 1 (15/13) no more epiphora and satified result; group 2 (15/33) diminished epiphora with satified result; group 3 (2/33) was noted to perform the second procedure with good result. All of the patients satified the results and no side effect such as wound infection and dry eye. Carbon dioxide laser is recommended for treatment of punctal stenosis.
