Measuring the Thickness of the Retinal Nerve Fiber Layer of Normal Subjects Using a Scanning Laser Polarimetry and Assessment of Its Reproducibility

目的:使用雷射掃瞄式偏極計測量正常人視網膜神經纖維層的厚度並且經由變異係數的分析比較雷射掃瞄式偏極計資料再現性的好壞。 方法:共有二十個自願者的四十個眼睛參與了這個實驗,我們使用雷射掃瞄式偏極計分別測量了一次他們視神經盤周圍四個區域(上側:30-150度,鼻側:150-210度,下側:210-330度,顳側:330-30度)內距離視神經盤邊緣一到一個半視神經盤直徑之寬度間視網膜神經纖維層的平均厚度。並從這些受試者之中隨機選擇出五位自願者,讓他們在不同的時間內接受五次雷射掃瞄式偏極計的測量,再用得到的資料計算出變異係數,以比較這部儀器之數值再現性的好壞。 結果:這些正常人視網膜神經纖維層的平約厚度在上側,下側,鼻側,顳側分別是0.1183mm±0.0158mm,0.1242mm±0.0163mm,0.0893mm±0.0097mm,0.0928mm±0.0114mm。而四個區域的變異係數分別是9.3±2.1%,9.5±2.5%,6.8±1.9%,7.5±2.6%。 討論:這部雷射掃瞄式偏極計在資料的再現性方面表現的很不錯,而在所測量的視網膜神經纖維層厚度資料中,我們可以發現在視神經盤的上側與下側的視網膜神經纖維層厚度是最厚的,而在鼻側與顳側則較薄。

Objective: We used a scanning laser polarimetry to measure the thickness of the retinal nerve fiber layer of normal subjects and evaluated the reproducibility of the scanning laser polarimetry obtained by analyzing their coefficients of variance. Method: Twenty normal subjects volunteered 40 eyes for participation in this study. Using a scanning laser polarimetry, we measured the average thickness of four sectors of the retinal nerve fiber layer around the optic disc between an area 1 and 1.5 disc diameters away from the margin of optic disc (superiorly: 30-150 degrees, medially: 150-210 degrees, inferiorly: 210-330 degrees, laterally: 330-30 degrees). Furthermore, 5 volunteers were randomly chosen from these normal subjects to undergo 5 measurements with the scanning laser polarimetry at different times. Coefficients of variation were calculated for the measurements obtained using the scanning laser poilarimetry to evaluate the reproducibility of results. Results: The average thickness of the retinal nerve fiber layer in these normal subjects was 0.1183±0.0158mm superiorly, 0.1242±0.0163mm inferiorly, 0.0893±0.0097mm nasally, and 0.0928±0.0114mm temporally. The coefficient of variation was 9.3±2.1% superiorly, 9.5±2.5% inferiorly, 6.8±1.9% nasally, and 7.5±2.6% temporally. Conclusion: This scanning laser polarimetry generated good reproducibility. From the measurements obtained, we observed that the retinal nerve fiber layer was thickest superiorly and inferiorly to the optic disc, and thinner medially and laterally.
