Retraction of lower eyelid occurred frequently with previous inferior rectus muscle recession surgery. We performed an adjustable lower lid traction technique to prevent such retraction. The technique involves an adjustable suspension of the capsulopalpebral head of the inferior rectus muscle from the original IR insertion at the time of strabismus surgery. Besides, the normal lid function was preserved, and the lid position can be adjusted separately at the time of muscle adjustment. We reviewed 39 cases (1992-1996) who received IR recession. 29 eyes in 29 patients received lower lid traction suture as well as IR recession. 10 eyes in 10 patients as control group. About 83% of the patients showed no lower lid retraction with the technique done; whereas only 50% in control group had no lid retraction. The mean lid retraction is -0.13 mm in study group and.0.30 mm in control group (P<0.05). In those who remained minor retraction, Graves' disease is the majority. Our study confirmed the effectiveness of this adjustable lid traction suturing to prevent lower lid retraction.
