Ocular Manifestations Secondary to Strangulation Injury: A Case Report
勒頸窒息後之眼部病變
摘要
目的：報告一個年輕男性遭受父親扼掐頸部，導致窒息傷害後的眼部臨床表現。 方法：病例報告。 結果：一位年輕男子被扼掐脖子後就醫，主訴視力模糊、視線變暗。眼科檢查發現嚴重結膜下血腫併視網膜水腫、臉部及眼皮紅腫合併出血性紅斑。其他檢查發現眼壓正常，無前房出血。血液生化檢查除了紅血球數降低，發現輕微血鈉降低，其他結果正常。兩星期後，其結膜下出血完全吸收，眼皮消腫，紅斑消失，視力已完全恢復。 結論：扼掐頸部造成的眼部表現臨床上極為少見。因外力造成暫時性缺氧與靜脈回流的阻斷，眼科醫師應注意可能的併發症包括結膜下出血，瀰漫性視網膜出血，眼外肌出血，視網膜血管阻塞，眼壓升高等。此病例表現出窒息倖存者的眼部傷害。

Purpose: To report an unusual case of bilateral subconjunctival hemorrhage with diffuse petechia all over the face in a survivor who suffered from a strangulation attempt. Methods: Case report. Results: A young man came to the emergency room with the complaint of blurred vision after a sudden strangulation attack by his father. The skin of his face showed diffuse petechial hemorrhage, both eyelids were ecchymotic and severely edematous. The slit lamp examination revealed severe bilateral subconjunctival hemorrhage. The anterior segments of both eyes were normal. Indirect ophthalmoscopy revealed retina whitening with poor red reflex otherwise the retinal vessels and optic discs were normal. The visual acuity and intraocular pressure were within normal limits. Laboratory data showed erythropenia and decreased hematocrit, and electrolyte showed mild hyponatremia. The subconjunctival hemorrhage completely resolved and skin petechial patches resolved in two weeks. Conclusion: Ophthalmologists should be aware of potential complications including hemorrhagic retinopathy, extraocular muscle hemorrhage, retinal vessels occlusion and elevated intraocular pressure following strangulation.
