目的:報告一例患有兩側非動脈炎性前缺血性視神經病變(Nonarteritic anterior ischemic neuropathy(NAION))的七十歲男性,接受玻璃體內注射Kenacort-A的治療成效。 方法:在玻璃體內注射4 mg/0.1 ml Kenacort-A到右眼,而左眼並未接受任何治療。 結果:接受玻璃體內注射Kenacort-A的右眼,其視力和視野都有明顯的改善,而沒有接受治療的左眼,其視力和視野皆進一步的惡化。 討論:在玻璃體內注射Kenacort-A,形成局部高壓濃度的類固醇,而降低因缺血所造成的視神經盤水腫,且更進一步地抑制因視神經腫大所造成的視神經盤水腫,且更進一步地抑制固視神經血液循環不足的惡化。但此治療成效,仍需要更廣泛的臨床病例才能加以證實。
Purpose: To report a case of bilateral nonarteritic anterior ischemic optic neuropathy (NAION) and evaluate the effect of intravitreal triamcinolone acetonide injection on the treatment of NAION. Method: Intravitreal injection of 4 mg Kenacort-A was performed in one eye with NAION. The fellow eye did not receive any treatment. Visual acuity, visual field, and color fundus photography were performed in both eyes. Results: Disc edema was partially resolved and improvement in visual function was noted 4 days after intravitreal injection of Kenacort-A in the right eye. Both visual field and visual acuity furthere deteriorated in the fellow untreated eye. Conclusions: Intravitreal injection of triamcinolone acetonide might eliminate disc swelling and terminate the vicious cycle of scleral outlet compartment syndrome in NAION. Randomized clinical trials of a larger sample are necessary to confirm the effect of the treatment.
