以翼狀贅肉呈現的結膜鱗狀細胞癌－病例報告
Conjunctival Squamous Cell Carcinoma Masquerading as a Pterygium
摘要
目的：報告一例以翼狀贅肉呈現的結膜鱗狀細胞癌。 方法：病例報告。 結果：一位85歲男性病患，因右眼發紅以及異物感持續一個月而求診。眼部檢查發現右眼角膜的鼻側有一個翼狀贅肉，在翼狀贅肉的前端有一個隆起的白色腫瘤。病患的翼狀贅肉在手術顯微鏡下被整個切除，病理切片報告顯示翼狀贅肉上的腫瘤是個分化良好的乳突形態結膜鱗狀細胞癌。為了避免術後結膜鱗狀細胞癌的復發，病患的右眼接受兩星期的Mitomycin C藥水局部點眼治療。病患術後狀況良好，至今13個月的追蹤，並沒有出現腫瘤復發，也沒有發生併發症。 結論：翼狀贅肉與結膜鱗狀細胞癌兩者都與長期紫外線照射有關，本文報告一例發生於翼狀贅肉上之結膜鱗狀細胞癌。對於異常之翼狀贅肉，我們將懷疑是否為結膜鱗狀細胞癌的可能性。

Purpose: To report a case of squamous cell carcinoma of the conjunctiva. Materials and methods: Case report. Results: An 85-year-old male patient suffered from redness and foreign body sensation of his right eye for about one month. Ocular examination showed a pterygium over the nasal side of his right eye. On the head of the pterygium, there was an elevated leukoplakic mass. Excision of the pterygium was performed and the histopathology revealed well-differentiated squamous cell carcinoma. The patient was treated with topical Mitomycin C 0.02% three times daily for two weeks to prevent recurrence. Conclusions: Both pterygium and ocular surface squamous neoplasia are associated with exposure to ultraviolet light. We present this case to show the importance of being alert for squamous cell neoplasia in any atypical pterygium.
