目的:比較常見4種人工水晶體計算公式SRK-Ⅱ,SRK-T,Hoffer Q,Holladay 1之準確度。研究方法:病例回溯性研究87隻眼睛在進行無併發症之白內障手術並植入人工水晶體後之球柱面度數總和之前後差異,病患眼軸長介於21.1mm至36.89mm,由同一位手術醫師執行。將病患依眼軸長分別分為三組,手術前預測值與術後實際值差異愈小者為愈準確。結果:在術後至少追蹤半年的時間內,在長、短眼軸長組別中因樣本數少而無法進行有效之統計分析,而以全部(87隻眼睛)進行變異數分析(ANOVA),4個計算公式間並未達到顯著差異。結論:以卡爾蔡司光學測量儀(IOL master)測量人工水晶體度數,四個公式中並無任一公式是最準確的。
Purpose: To compare the accuracy of the intraocular lens ( IOL ) power calculation formulas : SRK-Ⅱ, SRK-T, Hoffer Q, and HolladayⅠ. Method: This retrospective study included 87 eyes with axial lengths (AL) varying between 21.1 mm to 36.89 mm in which uncomplicated phaco-emulsification with PCIOL implantation was performed by one surgeon. They were stratified into three groups according to AL (<22.5 mm, 22.5 to 25.5 mm, and >25.5 mm, respectively). The predictive accuracy of the four formulas was analyzed by comparing the actual and predicted postoperative spherical equivalence (SE). Result: In the short and long AL group, due to insufficient case numbers and large variation, we could not analyze the data in a statistically meaningful manner. Generally, all formulas showed a mild myopic shift in prediction, but there was no statistical difference between the four formulas. There were over 90% cases estimated within 1 diopter(D) deviation from the predicted post-operative refractive power with the Hoffer Q, and HolladayⅠformulas. Conclusion: No formula was more accurate than the others in using the Carl Zeiss IOL master to calculate PCIOL power.
