目的:利用非侵入性測量的淚膜破裂時間以比較兩種不同含水量的隱形眼鏡對於淚膜穩定度的影響。方法:我們收集三十位患者隨機分配為兩組,分別配戴日拋的 Etafilcon A hydrogel (58% watercontent)隱形眼鏡或者月拋的Polymacon hydrogels(38% water content) 隱形眼鏡,並且測量配戴隱形眼鏡之前、戴後十五分鐘、四小時、及八小時的非侵入性測量的淚膜破裂時間。結果:非侵入性測量的淚膜破裂時間在配戴Etafilcon A hydrogel 的使用者是戴隱形眼鏡之前為8.335.0 秒、戴後十五分鐘為6.03±3.0秒、四小時為5.3±2.56秒、及八小時為4.53±2.36秒。配戴Polymacon hydrogels 使用者的非侵入性測量的淚膜破裂時間,戴隱形眼鏡之前為7.93±2.50秒、戴後十五分鐘為6.0±2.41秒、四小時為5.93±2.38秒、及八小時為5.53±2.18秒。可以發現兩組的非侵入性測量的淚膜破裂時間均隨著時間增加有顯著的下降 (p <0.01)。但是非侵入性測量的淚膜破裂時間在配戴含水量高的隱形眼鏡(Etafilcon A hydrogel)使用者有更明顯的下降。結論:隱形眼鏡的配戴對於淚膜穩定度有明顯的影響。不同含水量的隱形眼鏡會影響眼睛表面的潤濕性,會造成使用者在整天配戴隱形眼鏡後的乾眼症,並且導致他們停止配戴隱形眼鏡。
Purpose: to compare the tear film stability of two hydrogel soft contact lenses by using non-invasive tear film break up time (TBUT) assessment. Methods: Non-invasive TBUT (NIBUT) were measured on 30 myopic contact lens wearers randomly fitted with daily Etafilcon A hydrogel (58% water content) or monthly Polymacon hydrogels (38% water content). Time point measurements include before contact lens wear, 15 minutes, 4 hours, and 8 hours after contact lens wearing. Result: The NIBUT in Etafilcon A hydrogel users was 8.335.0 seconds at pre-wear, 6.03 3.0 seconds at 15 minutes, 5.3±2.56 seconds at 4 hours, and 4.53±2.36 seconds at 8 hours of SCL wearing time. In Polymacon hydrogels users, NIBUT was 7.93±2.50 seconds at pre wear, 6.0±2.41 seconds at 15 minutes, 5.93±2.38 seconds at 4 hours, and 5.53 seconds ±2.18 at 8 hours of SCL wearing time. There were significant decrease in the NIBUT with extended lens wear for both groups (p <0.01). However, the NIBUT drop was significantly higher in patients who were fitted with higher water content contact lens (Etafilcon A hydrogel). Conclusion: Contact lenses induced significant changes in the tear film. Differences in the water content of hydrogel contact lenses may affect the wettability of the ocular surface which is the main factor causing end of day dry eye symptoms and contact lens drop-out.
