南非航空SA-295之失事調查聽證經驗
Accident Investigation and Hearing Experience of the Helderberg Air Disaster
摘要
一九八七年十一月二十八日南非航空中B-747 COMBI型客貨兩用機，搭載140位乘客（含30位日籍及7位中國旅客）及19位組員，在模里西斯島東北方134浬處，因空中起火墜海失事。南非政府隨即組成跨國調查委員會進行失事調查，經年餘之調查後，舉行公聽會，終使失事真象大明。 文中特別介紹南非政府此次失事調查之組織架構、成員特質、作業流程方式，尤其是反復交替驗證檢查之作法，力求公正客觀，以明失事真相，他山之石可以攻錯，值得我們省思。 文中亦就南非航空SA-295班機失事醫學調查部份，由遺骸勘驗、解剖、病理、毒物報告。航空醫學之研判分析，進一步事故之重演，正確的掌握了失事當時的狀況，也說明了航空醫學對失事調查的重要性！

In Nov. 28, 1987, the South African Airlines B-747 COMBI which was a combined passenger and cargo plane, carried 140 passengers (included 30 Japanese nationals and 7 Chinese travellers) and 19 staffs. When the aircraft was at North East of Mauritius Island, nautical mile of 134, met an accident as fire erupted in mid air and caused the aircraft to drop into the sea. The South African Government immediately set up International Investigation Committeee to investigate the matter, after two years conducted a public hearing meeting and eventually the true facts of the accident was known. This essay specially introduces the investigation work of the South African Govenment, their constuction work, the special characteristics of members formation, methods of work procedures, and specially their repeated cross examination work, striving for a fair objective and uncovered the true facts of the accidents. Learning from these can fight wrong and worth for us to make a self examination of our thoughts. This essay is also a part of the South African aircraft SA-295 mishap medical investigation, from inspection and examination of the dead bodies, autopsy, pathology, toxicological report. Judgement and analysis of aviation medicine, further reconstruction of aircraft mishaps, perfectly control the situation on spot, and also describes the important role that aviation medicine plays in aircraft mishap investigation.
