由休克病患耳垂取血液標本測得血糖值之凖確度

Accuracy of Blood Glucose Value from Ear-lobe Blood Sample in Shock Patients

本研究以外科加護中心之休克病患為對象(n=35),由病患耳垂、指尖及動脈導管取血液標本,分別以攜帶式血糖儀測其血糖值,並探討由耳垂取血液標本所測得血糖值之率確度,以了解其於臨床應用之可行性,期能提供一變通之方法,率確監測血糖。研究結果顯示,耳垂血液標本利得之血糖值(139.94士70.61)與動脈血液標本所測之血糖值(141.48士70.62)於統計學上並無顯著差異。指尖血液標本所測之血糖值(131.43士70.66)則顯著低於動脈血液標本及耳垂血液標本(P=0.001)。由此推論,臨床上,低血壓之病患,不適合由指尖札取血液標本監利血糖值;自耳垂取血液標本可凖確監測血糖值。

To evaluate the accuracy of ear-lobe glucose value in shock patients. A prospective, nonrandomized comparison study design was used. Blood was withdrawn simultaneously from ear-lobe, fingertip, and arterial line among 35 shock patients in SICU during a twelve-month period. Data obtained by four methods of glucose analysis (ear-lobe reagent strip, fingertip reagent strip, arterial reagent strip, and laboratory arterial blood glucose values) were compared. Statistical method of paired t-test was used to compare significant difference between these data. The mean fingertip blood glucose value (131.43±70.66) was significantly lower than the values measured either by ear-lobe reagent strip (139.94±70.61) or arterial reagent strip (141.48±70.62) (p=0.00l). There was no significant difference between either ear-lobe reagent strip or arterial reagent strip glucose values and laboratory arterial blood glucose values (142.37±68.66). The results suggest that fingertip blood sample should not be used routinely for blood glucose analysis in hypotensive patients. Sampling from ear-lobe provides more accurate information related to blood glucose value in shock patients when compared with fingertip capillary data.
