Life-threatening Endocrine Emergency-Thyroid Storm: A Case Report and Review
摘要
本案例為52歲男性，主要症狀為體重減輕、嗯心、嘔吐、腹瀉、咳嗽有疚、心悸、姿燒及意識變化。心電圖檢查為心室上心轉過速及心房顫動合併心室快速反應，心臟超音波檢查為心臟運動過強，甲狀線功能檢查顯示甲狀腺功能亢進。診斷為甲狀腺風暴後，接受內科治療，恢復良好。 甲狀線風暴就是指足以致命的甲狀線功能亢進、同時合併有一個或多個器官功能失償(decompensa-tion)的狀態，死亡率高。目前無特定的診斷標準，以臨床判斷為主。致病機轉目前不十分明瞭，許多狀況均可誘醫此病，治療需同時針對風暴本身及併存之誘發因子，早期診斷為獲得良好療效之關鍵。

A 52-year-old male presented with the symptom of body weight loss, nausea, vomiting, diarrhea, cough with sputum, and palpitation, followed by fever and consciousness change. EKG showed, paroxysmal supraventricular tachycardia(PSVT) and atrial fibrillation with rapid ventricular response. Echocardiogram showed hyperkinetic heart. Thyoid hormone level was higher than normal. The thyroid storm was impressed and then medical treatment was prescribed. The patient recovered well later. Thyroid storm is a life-threatening exacerbation of hyperthyroid state, complicated with the evidence of decompensation in one or more organs, and associated with a high mortality rate. There is no uniform criteria to diagnosis of thyroid storm. It is made clinically. It’s pathogenesis has not been fully elucidated now. A wide variety of factors could be as precipitating events, and should be treated as well as thyroid storm. Effective management depends on early recognition of thyroid storm.
