成功急救甲狀腺風暴併突發性心臟停止案例緊急處置與良好成效：兩病例報告

摘要

甲狀線疾病病人發生甲狀腺毒性並不常見，常以三十至六十歲女性為主。而甲狀腺毒性心臟衰竭及低血壓等嚴重併發症會導致突發性心臟停止。我們描述兩位甲狀線風暴合併甲狀腺毒性心臟衰竭病人。在這兩個病例中，一位病患曾經接受次全性甲狀線切除術併發嚴重肺水腫。而另一位病患有糖尿病病史合併酮酸中毒。此兩位病患因甲狀腺毒性心臟衰竭併發突發性心搏停止。經我們正確診斷及治療後，兩位病患成功地迅速恢復健康。

A thyroid storm is a rare manifestation of thyrotoxicosis and usually occurs in females during the third to sixth decades of life. Serious complications such as heart failure and hypotension resulting from cardiac arrest may occur. We describe two cases of young females who suffered from thyroid storms with thyrotoxic cardiac failure. Severe pulmonary edema and respiratory failure presented in the first patient who had previously received a subtotal thyroidectomy. Decompensated diabetic ketoacidosis presented in the second patient. Both of them were complicated by sudden cardiac arrest and the patients were successfully resuscitated with a good outcome after aggressive management.
