背景:對於注重社會福利的國家而言,聽力鑑定在殘障鑑定及體住鑑定的工作是非常重要的。目前我國以平均純音聽力閾值作為鑑定的標準,配合腦幹聽性反應作為客觀標準。然而此兩種檢查分別為主客觀檢查無法直接對比,因此本研究嘗試在最少外界誘因影響下尋找此兩種檢查之相關性。 方法:以突發性耳聾患者之優耳為研究組,尋找純音聽力檢查各頻率閾值與腦幹聽性反應閾值的相關性,並進而推導出線性公式。再將此線性公式套用於殘障鑑定組患者,藉以評估此線公式的可用性。 結果:研究組中純音聽力檢查4000Hz閾值與腦幹聽性反應閾值相關性最佳,且其線性相公式為y=0.501x+31.475。鑑定組腦幹反應閾值平均比預測腦幹聽反應閾值要高出近10dB。 結論:純音聽力檢查4000Hz閾值與腦幹聽反應閾值存在線性相關。建議每個鑑定單位應利用自己的資料推導適合的公式,可能對臨床殘障鑑定工作會有所幫助。
BACKGROUND: As an otolaryngologist, hearing threshold detection is one of major the works. The behavioral pure-tone audiometry (PTA) combined with objective auditory brainstem response (ABR) are thought as ”gold standard” examinations for hearing threshold detection, but the relationship between PTA and ABR is still unclear. So we try to find a way to explain the relationship between PTA and ABR. METHODS: We use the better hearing ear of sudden deafness patients as our study subjects to find the relationship and linear equation between PTA and ABR. This equation was used in disability-certificated patient in order to evaluate its accessiblility. RESULTS: The correlation between PTA at 4000Hz and ABR is better than other frequencies. Linear equation is y=0.501x+31.475. The average ABR threshold is almost 10 dB higher than predicted ABR threshold in disability-certificated group. CONCLUSION: There is a linear correlation between PTA at 4000Hz threshold and ABR threshold. The proper equation is helpful in clinical hearing threshold detection.
