勞工醫院如何建立床邊血液氣體分析之品質系統
How to Establish the Quality System for Bedside Blood Gas Analysis in Labor Hospitals
摘要
床邊血液氣體分析在勞工醫院急診或重症疾病是相當重要的檢驗項目之一。臨床血液氣體分析非護理人員之專業領域，且護理部門管理人員都無血液氣體分析品質管理與標準作業之專業訓練，及加護病房、開刀房、急診室的血液氣體分析都由護士或醫師執行，因此勞工醫院應建立一套床邊血液氣體分析品質系統，降低檢驗誤差以提昇分析品質。ISO 15189為國際間醫學實驗室認證之標準規範，包括品質管理要求與技術作業要求，可提昇實驗室的效率與功能。勞工醫院應建立床邊血液氣體符合ISO 15189規範之要求，可提供勞工醫院設置床邊血液氣體分析品質系統之作業指引，並符合醫院評鑑之要求及提昇病人安全。

Bedside blood gas analysis in labor hospital emergency room or intensive disease is one of the very important test items. The blood gas analysis is not a specialization of regular nurses, and the nursing personnel and management do not have the standard operation procedures and professional training on the blood gas analysis quality control, all the blood gas analysis in ICU, operation rooms, and ER are done by nurses or doctors. Therefore the nursing department should establish a bedside blood gas analysis quality management and standard operation procedure, in order to reduce errors and increase analysis quality. ISO 15189 is the regulatory standards certified by medical laboratories internationally, including quality management and technical requirement, in order to improve the effectiveness and functions of the laboratory. The labor hospital should establish an ISO 15189 approved bedside blood gas analysis procedures and standards to provide guidelines for the bedside blood gas analysis quality in the nursing department. These guidelines need to also confirm the requirements set by the labor hospital in order to enhance the patient’s safety of emergency.
