代謝症候群及血液相關生化指標與血管內皮功能之表現

The association between metabolic syndrome, the associated biomarkers, and endothelial function

摘要


本研究的目的是探討代謝症候群與血管內皮功能之相關性。本研究以台北市某醫學中心參與年度員工體檢的員工為對象,以方便取樣方式進行收案。資料收集內容包含基本人口學資料、身體檢查及生化血液檢查結果與血管內皮功能測試結果。代謝症候群的定義用國健署代謝症候群臨床診斷標準,血管內皮功能是以血管內皮功能偵測儀(Endo PAT2000)作相關測試。本研究共收案53人,平均年齡35.8± 7.7歲,女性占72%。收案對象代謝症候群比例為11.5%。各項代謝症候群定義中所佔比例最高者為「腰圍過粗」(29.4%),其次為「血壓過高」佔28.3%及「高密度膽固醇過低」佔25%,再其次為「三酸甘油脂過高」(17.9%)及「空腹血糖過高」(3.9%)。有代謝症候群受試者血管內皮功能的表現與無代謝症候群受試者相比,血管彈性係數(AI)及75下心跳標準化血管彈性係數(AI@75bpm)皆較高,但其間的差異性以及線性迴歸分析結果,皆無達到統計上明顯差異。本研究各項分析雖無達到統計上明顯差異,但仍有符合代謝症候群數目較多者,於血管內皮功能上,表現出高心血管疾病相關的血管硬化程度增加。本研究初步探究了代謝症候群對血管內皮功能的影響,可作為後續研究的參考。

The metabolic syndrome, a clustering of traditional cerebrovascular disease (CVD) risk factors, is an example of a prevalent and treatable condition. Even though there is ongoing dispute about its definition and concept, the metabolic syndrome does increase the risk of diabetes and cardiovascular morbidity and mortality. Early identification and intervention of the metabolic population is related to the prevention from illness. In this study, we included 53 hospital crews with above 20 years old of age in a regular health check-up from 2015 to 2016. We tried to find the proportion of metabolic syndrome people in the hospital, and the correlation between metabolic syndrome and endothelial function by using the peripheral arterial tonometry (Endo PAT2000). The results showed that the proportion of hospital crews with metabolic syndrome was 11.5 %. According to the definition of metabolic syndrome criteria, the highest proportion of presentation in metabolic syndrome was "abdominal obesity" (29.4%), and followed by "high blood pressure" (28.3%). Compared with the "non-metabolic syndrome" group, the "metabolic syndrome" groups showed higher augmentation index (AI), but the differences between groups and the linear regression analysis all lacked of statistically significant. Although there was no statistically significance in this study, weak relevant was still presented in the association between metabolic syndrome component and endothelial dysfunction, with the presentation of the cardiovascular disease and vascular stiffness related higher AI.
