Miliary tuberculosis (TB), an often-fatal infectious disease in the absence of intervention, is increasingly observed among immunocompromised individuals. This case study presented a middle-aged man with rheumatoid arthritis who developed miliary TB following anti-tumor necrosis factor-alpha antibody therapy. Sputum conversion was achieved 3 months after the initiation of therapy, but the patient subsequently experienced TB-immune reconstitution inflammatory syndrome characterized by multi-organ involvement.
