摘要


氨氣吸入性傷害,常常發生在低溫壓縮機系統故障漏氣,由於氨氣極具水溶性,所以常常造成吸入性肺部傷害及眼睛的刺激,嚴重的會造成肺水腫甚至死亡。我們發現除了這些問題外,患者受傷時,血液中的氨含量會隨嚴重度而升高,但在數小時後便降到正常值以下。此外肝功能指數會漸漸升高,直到受傷後第八天達到最高峰,這在其他文獻是沒有報告過的。我們整理了相關文獻報告,希望用血液中氨的含量來評估患者一開始的嚴重度,且要追蹤肝功能指數兩週以上,並及時給予支持性療法,以降低患者的合併症。

When encounter ammonia inhalation injuries we often focus on oropharynx or tracheal mucosa lesion and lung parenchyma injury, and neglect of other systemic injuries. In our study, we found that the abnormal liver function tests proceed until post injury day 8. It might be an useful study to determine if the liver function tests could be an indicator of the severity of injury and prognostic factor. Besides, the initial serum ammonia level may be a meaningful indicator of severity of injury even it could be fade to normal in 4-5 hours. Reviewing the literatures we finding that close follow up of liver function tests in ammonia injury is necessary.
