Upper Eyelid Gold-Weight Implant for Lagophthalmos in Facial Paralysis
上眼瞼金片植入術用於治療顏面麻痺所導致的兔眼
摘要
顏面神經麻痺會導致功能性以及美觀上的障礙，包括眼睛無法閉合（兔眼）、眼淚分泌減少、眨眼功能不全以及臉部的不對稱。這些病人可能會有暴露性角膜炎、角膜潰瘍甚至失明的危險。矯治兔眼有許多非手術及手術的方法，上眼瞼金片植入術用於矯治兔眼是一種已經被確立，且具有成功療效的方法。但是，它也產生了許多併發症，例如：植入物的移位或突出、感染、兔眼矯正失敗及術後眼瞼過於下垂。這篇文章中，我們提出改良式更薄而面積大的植入金片，可以避免這些併發症。此外，在美觀上也達到看不出與摸不著有植入外物的效果。

Facial nerve palsy results in functional and cosmetic disabilities, including inability to close the eyelids (lagophthalmos), decrease in tear production, incomplete blinking, and asymmetric looking. These patients are at greater risk of exposure keratitis, corneal ulceration and even blindness. There are a variety of non-surgical and surgical options for correcting lagophthalmos. The use of gold weight eyelid implant is an established procedure in treatment of lagophthalmos with successful results. However, complications with these methods continue to occur (implant migration or extrusion, wound infection, failure to correct the lagophthalmos, and excessive postoperative ptosis). This study describes our modification of thinner, longer and wider gold weight implant in order to prevent these complications, and moreover to achieve a more satisfying cosmetic result.
