背景:罹患肢體多發性脂肪瘤的病患很在意因為脂肪過多而導致外形上的明顯畸形,尤其當這些脂肪長在病患裸露的上肢上會更明顯。雖然多發性脂肪瘤是無痛、良性的,但是不舒服感及畸形的外觀,讓病患想將這些脂肪瘤移除。 目的及目標:傳統的手術方式,一個切口移除一個脂肪瘤,會造成數不清的手術切口及留下術後不好看的外觀。然而其他較少畸形及侵入性的方式,如脂肪吸除手術,另加/不加內視鏡的輔助等,也有相關的缺點。於此,我們採用最少侵入性手術方式來克服這些問題。 材料及方法:我們利用最少侵入性方式,經由最少的手術切口設計來局部脂肪切除;並以一個上肢多發性脂肪瘤的病例來詳細示範我們的作法。 結果:所有的脂肪瘤能經由最少的手術切口設計來成功切除,且沒有產生任何併發症或後遺症。經過平均六個月到一年的術後追蹤,病人都很滿意術後的結果。 結論:以我們的作法,能容易地完整切除上肢的多發性脂肪瘤,並且儘可能獲得術後既美觀又令人滿意的結果。另外,這些手術切口最終會變成可再利用切口,日後有新生成的脂肪瘤就不用再創造新的切口來處理。
Background: The development of lipomatosis over the extremities can be distracting to affected individuals because of their deforming conspicuousness, this being especially the case for those where such lesions occur over the denuded upper extremities. Although lipomatosis are painless and pathologically benign, discomfort and disfigurement encourage these patients to seek the removal of such lesions. Aim and Objectives: The traditional surgical method of lipomatosis removal through an incision can lead to the need for countless incisions and significant post-operative disfigurement. However, the other alternatives in less-deforming and less-invasive approach, such as liposuction with/without the addition of an endoscope etc, are also associated with some drawbacks. Herein, we employed the minimally invasive technique to overcome these problems in treating such cases with surgery. Materials and Methods: We demonstrated the minimally invasive technique of regional lipectomy via remote incisions in detail and illustrated this technique with a case suffering from lipomatosis over upper extremities. Results: All lipomas were successfully removed via remote incisions and there was no significant complication or sequelae postoperatively. All our patients were satisfied with the results of their surgery as being assessed at the six-month to one-year follow-up. Conclusions: With our techniques, it is easily achievable to excise lipomatosis of the upper extremities completely and to btain as aesthetically pleasing results as possible. In addition, these incisions eventually became reusable incisions to facilitate the future management of any newly emerging lipomas without the need to create new incisions.
