定量評估中風患者之肌力與功能狀況相關性的分析

摘要


中風之罹患率至今仍高,存活者皆須復健治療,其肌力恢復狀況常因伴隨痙攣很難客觀評估,本文研究之目的,乃在於客觀評估中風患者之肌力並分析其與功能狀況之相關性,研究的對象是選取1989年問卷調查之中風患者中屬於第一次且單側中風半年以上者,總共有53位接受等速運動儀器(The Medizin-Electronic System GmbH, Isokinetic hydrolic drive GGT-3000)之肌力定量分析,測試之肌肉?包括健側與患側之肘屈肌與伸肌,膝曲肌與伸肌,測試之速度有10°/s與120°/s (採用被動模式),60°/s (測向心與離心收縮),結果顯示:健側肘曲肌離心收縮為向心收縮之兩倍,伸肌亦然;膝曲肌之離心收縮為約向心收縮之1.5倍,膝伸肌之離心收縮約為向心收縮之2.5倍,肌力與功能狀況如能否翻身,由平躺至坐,由坐至站,穿脫衣服等做相關統計分析,結果顯示:患側肘曲肌之向心及離心收縮之肌力,肘伸肌之離心收縮之肌力以及膝曲肌之離心收縮肌力對日常生活功能之恢復具有決定性之意義。

The purposes of this study were to quantitatively evaluate the muscular strength and spasticity of stroke patients and to find out the significant relationships between different functions and torques of muscle groups in mode and speed specificity of isokinetic evaluation. This study was to use isokinetic hydralic drive machine (GGT-3000, The Medizin-Electronic System, GmbH, West Germany) to measure quantitatively muscular activities of elbow and knee flexor and extensor muscular groups of 53 patients. Affected side was measured in passive motion at speed 10°/s, 120°/s and concentric contraction and eccentric contraction motion at speed 60°/s. Sound side was measured in concentric and eccentric contraction motion at speed 60°/s.Unpaired t-test and Nonparametric ANOVA were used to analyze all the datum. From this study, we concluded that quantitative evaluation of affected side elbow flexor concentric contraction torque at the speed 60°/s was highly suggested as an adequate method to determining the severity of spasticity of the stroke patients. The results also showed an important application for therapy and target torques to aim for in strengthening specific muscle groups for particular functions.
