落實正確的環境消毒是確保病人醫療安全重要的一環,清潔消毒未確實亦會增加照顧感染的風險,故引發此專案改善動機。為瞭解清潔人員執行病室環境清消情形,使用觀察表及實際訪談發現,病房清潔人員執行病室環境清消正確率僅62.6%。原因為:漂白水泡製量杯刻度認知錯誤、量杯刻度不佳、政策宣導未落實、查核工具未具體量化等。在2020年7月1日到10月10日間,專案小組更換全院漂白水量杯標示、宣導及張貼病室環境終消十步驟、辦理專案改善宣導會議、張貼環管天地公佈欄並宣導正確使用清潔工具,讓清潔人員能正確執行環境清潔消毒步驟,進而提高正確率。經上述措施執行後,清潔人員對環境清消正確率由62.6%提升至86%,顯示透過量化數據提供即時的監測及回饋,可提供安全衛生的環境以保障工作人員、病人及家屬的健康,期許可推廣到全台醫療院所,進而提升醫療環境品質。
Implementing correct environmental disinfection is an important part of ensuring patient medical safety. Uncertainty in cleaning and disinfection will also increase the risk of infection in patient care, which triggers the motivation for this project to make improve. In order to understand the cleaning staff's implementation of the ward environment cleanup, the use of observation checklist and in-depth interview found that the correct rate of the ward cleaning staff's implementation of the ward environment cleanup was only 62.2%. The reasons are: misunderstanding of the scale of the measuring cup made by bleaching water, poor scale of the measuring cup, unimplemented policy propaganda, and lack of specific quantification of the inspection tools. From July 1st to October 10th, 2020, the project team replaced the hospital-wide bleach measuring cup markings, publicized and posted the ten steps for the disinfection of the ward environment, handled the project improvement publicity meeting, posted the bulletin board of the environmental management and promote the correct use of cleaning tools allows cleaning staff to correctly perform environmental cleaning and disinfection steps, which improving the accuracy rate. After the implementation of the above measures, the accuracy rate of cleaning staff in cleaning the environment has increased from 62.2% to 86%, which Shows that providing real-time monitoring and feedback through quantitative data can provide a safe and hygienic environment to protect the health of staff, patients and their families. It is expected to be extended to all medical institutions in Taiwan to improve the medical environment quality.
