音樂治療於腎臟切片病人成效之系統性文獻回顧暨統合分析
Music Therapy in Renal Biopsy Patients: System Review and Meta-analysis
摘要
背景：腎臟病分為許多種類及階段，臨床上為了能確切治療或鑑別診斷腎臟病種類，需仰賴腎臟切片檢查，當病人面臨未知的治療時，內心不免產生心理上的變化，進而影響至生理指數變化，音樂治療為透過特定聲波，有計畫性、系統性的恢復與改善情緒，而當改善負向情緒同時增進正向情緒，但對於腎臟切片病人成效並不明確。目的：確認音樂治療於腎臟切片病人成效。方法：使用組織切片、切片、tissues biopsy、biopsy；音樂治療、music therapy；一般照護、usual care；緩解、減少、或改善切片造成的焦慮、疼痛及血壓升高為關鍵字搜尋，依據篩選標準進行篩選。結果：本系統性文獻回顧共納入七篇隨機對照試驗研究，並進行統合分析結果後發現，音樂治療於腎臟切片病人降低血壓達顯著差異（p=.03），而在疼痛（p=.07）及焦慮（p=.19）上無統計上差異，透過此推論音樂治療須每日至少播放一次，每次約20至30分鐘。結論／實務應用：整合文獻結果並應用於臨床案例中發現，腎臟切片病人可於入院當日即開始進行介入，並考量個案喜愛音樂進行聆聽至切片結束，方可有效降低血壓高情況。

Background:Kidney disease is divided into many types and stages. To accurately treat or differentially diagnose the types of kidney disease, it is necessary to rely on renal biopsy. When the patient faces unknown treatment, psychological changes will inevitably occur in the mild, which will affect the changes in physiological indexes. Music therapy is a planned and systematic recovery and improvement of mood through specific sound waves, and when it improves negative emotions and improves positive emotions, the effect onrenal biopsy patients is not clear. Purpose: Confirmation of the efficacy of music therapy in renal biopsy patients. Methods: Usetissue biopsy, biopsy; music therapy, music therapy; general care, usual care; alleviation, reduction, or improvement of anxiety, pain, and increase blood pressure caused by a biopsy to search, and screen according to the inclusion criteria. Results: A total of seven randomized controlled trials were included in this systematic review, and a meta-analysis found that music therapy in renal biopsy patients significantly decreased blood pressure (p=.03), pain (p=.07), and anxiety (p=.19) were not statistically different. It can be deduced that music therapy must be played at least once a day, about 20 to 30 minutes each time. Conclusion/ Implication for Practice: By integrating the literature results and applying them to clinical cases, it is found that patients with renal biopsy can start intervention on the day of admission and considering that the patient likes music and listen to the end of the biopsy, it can effectively reduce the high blood pressure.
