抗病毒藥Abacavir引發嚴重過敏反應之探討
Hypersensitivity Reactions Induced by The Antiviral Drug Abacavir
摘要
Abacavir為一種核苷類似物反轉錄酶抑制劑（nucleoside reverse transcriptase inhibitor），臨床上併用其他抗病毒藥物用以治療成人及兒童之人類免疫缺乏病毒（human immunodeficiency viruses）感染。然而abacavir可能會引起危及病人生命的過敏反應（hypersensitivity reactions），此abacavir過敏反應（abacavir hypersensitivity reactions, AHR）於2002年即證實與HLA-B*57:01基因具高度相關性。本文針對abacavir與其過敏反應、HLA-B*57:01基因與AHR之關聯，以HLA-B*57:01基因篩檢診斷或預防AHR之效益進行探討，期望abacavir在臨床使用上能更加安全而有效。

Abacavir is a nucleoside reverse transcriptase inhibitor (NRTI) used in the treatment of human immunodeficiency virus (HIV) infection. It is clinically used in combination with other antiviral medications as part of highly active antiretroviral therapy. Abacavir hypersensitivity reactions (AHR) may be severe and even life-threatening. In 2002, HLA-B*57:01 genotype was demonstrated to be strongly associated with AHR. We reviewed the association between abacavir, HLA-B*57:01 genotype and AHR to determine the benefits of HLA-B*57:01 genotype screening for diagnosing or preventing AHR, aiming the clinical management of abacavir safer and more effective in Taiwan.
