摘要
乾癬是一種慢性發炎的脫屑性皮膚疾病，在一般人口中約有0.1~3.0%受到影響。乾癬的指甲及關節侵犯並不罕見，然而乾癬指甲病變合併鄰近骨頭／骨膜侵犯則甚為稀少。我們的個案為乾癬指甲病變台併杵狀指，並無關節腔侵犯之證據，診斷為乾癬性關節炎裡面一個特別的分類即乾癬指甲－厚皮－骨周圍炎。本文做個案報告並回顧文獻。

Psoriasis is a chronic inflammatory scaling skin disorder affecting approximately 0.1~3.0% population. Nail and joint involvement are not uncommon in psoriatic patients. However, psoriatic onychopathy with an underlying bony/periosteal involvement in the distal phalanx is exceptionally rare. In our case, psoriasis with psoriatic nails and enlarged distal digits without evidence of joint involvement led to the diagnosis of psoriatic onycho-pachydermo-periostitis, which is recognized as a special entity in the spectrum of psoriatic arthritis. We reported this rare case and reviewed related literature.
