複雜性足癬黴菌病常常有合併趾縫間的綠膿桿菌感染,由於具抗藥性的綠膿桿菌逐漸增加,且許多病患足部的血液循環不佳,使得抗生素治療總是難以成功。過去使用包括醋酸(acetic acid),氯化苄二甲洞烴銨(benzalkonium chloride)溶液,或過錳酸鉀(potassium permanganate)溶液等的殺菌劑浸泡足部患處的治療策略往往沒有達到令人滿意的效果,因此使用這些殺菌劑的合理性有待評估。為了解這些過去使用的殺菌劑是否有效,並同時探索有效的新殺菌劑來治療複雜性足癬黴菌病,我們又加入了三種殺菌劑-次氯酸鈉(sodium hypochlorite)溶液、蜂膠(propolis),和普維酮-碘(povidone-iodine)溶液-來比較它們對綠膿桿菌的體外抗菌活性。我們研究發現普維酮-碘溶液和次氯酸鈉溶液在濃度0.125%以上有殺菌效果,使用它們治療複雜性足癬黴菌病似乎是大有可為的。醋酸的效果稍差,濃度0.125%以上就可以抑制綠膿桿菌生長;相反的,因為氯化苄二甲烴銨溶液、過錳酸鉀溶液,及蜂膠需較高的濃度才能抑制綠膿桿菌生長,所以可能不適合作為治療複雜性足癬黴菌病的殺菌劑。
Concomitant infection of Pseudomonas aeruginosa in toe webs is a rather common phenomenon in dermatophytosis complex. Because of the ever increasing resistant strains of Pseudomonas aeruginosa against antibiotics, and compromised circulation the affected patients frequently possess, the antibiotic therapies always face the challenge of winning the battle. Previous treatment strategies of soaking the affected feet in several antiseptics including acetic acid, benzalkonium chloride solution, or potassium permanganate solution did not yield satisfactory or consistent results. It remains to be determined whether the use of these antiseptics is justified. Therefore we added three candidate antiseptics-sodium hypochlorite solution, propolis, and povidone-iodine solution-to the treatment strategies and compared in vitro the antibiotic activities against Pseudomonas aeruginosa. We intended to know whether the antiseptics used in the past were effective and meanwhile exploring new potential antiseptics for the treatment of dermatophytosis complex. Our study showed that povidone-iodine solution as well as sodium hypochlorite solution with the concentration greater than 0.125% was bactericidal. The use of these two antiseptics in the treatment of dermatophytosis complex seems to be promising. Acetic acid was less effective. With the concentration greater than 0.125%, it could inhibit the growth of Pseudomonas aeruginosa (bacteriostatic). On the contraty, higher concentrations were needed for benzalkonium chloride solution, potassium permanganate solution, and propolis to inhibit the growth of Pseudomonas aeruginosa and they are probably not suitable candidate antiseptics for treating dermatophytosis complex.
