引導組織再生術(guided tissue regeneration, GTR)應用於牙周手術治療牙周疾病已有十多年的歷史。從一開始時所使用的不可吸收再生膜(non-absorbable membrane)到現在普遍使用的可吸收再生膜(absorbable membrane),其目的是在治療結果可預期的情況下,簡化再生手術的方法,讓患者與術者均蒙其利。眾所皆知,不可吸收再生膜在操作上確實是相當技術敏感的(technique sensitive)而且需要二次手術取出不可吸收的再生膜,而可吸收的再生膜則完全改善不可吸收再生膜的缺點。因此本報告的目的是從臨床的觀點來探討各種再生手術實際應用的效果比較進而預測未來的趨勢。本報告的內容將涵蓋國內所引進各種不可吸收與可吸收再生材料在牙周再生手術的臨床應用現況,包括操作難易度、優點與缺點、臨床軟組織表現及如何處理再生膜過早露出的問題等等。近年來引進牙釉基質衍生物(enamel matrix derivative, EMD)於牙周再生手術,讓牙周再生手術又向前邁進一步;另外由於基礎醫學及各種材料學不斷的進步,相信在不久的未來,我們將有操作簡單而且療效又好的牙周再生材料可供治療牙周病使用。
Guided tissue regeneration (GTR) with barrier membrane has been applied in the periodontal regenerative surgery for more that 15 years. From non-absorbable membrane to absorbable membrane, the predictable regenerative results and simplified surgical method are persuited. As we know, the major drawbacks of non-absorbable membrane are highly technique-sensitive technique and second surgery for removal of the membrane. The appearance of absorbable membrane has no above-mentioned drawbacks. This paper is to compare the difficulties between different membrane regeneration modalities from clinical point of view and predict the future trend in regeneration. Recently, a non-membrane regeneration material-enamel matrix derivative has been applied in regeneration surgery. The dream in searching a predictable regeneration material without sensitive technique will become true soon.
