植牙手術併發症的診斷、處理與預防
Surgical Complications of Dental Implants: Diagnosis, Management, and Prevention
摘要
本文主要探討牙科植牙手術可能產生的併發症，包括手術時的流血、神經傷害、高溫對骨癒合的影響、異物吞食、植體穩定性不足、覆蓋螺帽早期裸露以及手術感染等問題。這些問題發生的頻率或許不高，可是一旦產生，卻可能造成患者的不適、引發組織的炎性反應、影響植體周圍組織的癒合，嚴重者甚而導致植牙治療的失敗。因此植牙手術者必須了手術可能產生的併發症及其相關因素，才能有效防範問題的產生。植牙手術產生併發症的相關因素包括病患本身癒合能力差、植牙區的骨質疏鬆、手術醫師經驗不足、手術傷害或細菌感染等。本文除了討論這些併發症的診斷與處理方法外，並進一步說明如何在手術中預防這些併發症的發生。

The aim of this article is to review the surgical complications related to dental implant therapy, including hemorrhage, sensory disturbances, overheating on bone drilling, foreign body swallowing, loss of primary stability of implant, early exposure of cover screw and postoperative infection. Although the incidence of these complications is rare, the inadvertence occurrence may result in patient's discomfort, tissue inflammation, damage to peri-implant tissues, or even implant failure. To effectively reduce these untoward events, dental practitioners shoud realize the risks of surgical complications in implant therapy and their possible causes, such as those associated with patient factors that impair healing, poor bone quality, operator inexperience, surgical trauma, and bacterial contamination. In this review, useful diagnostic procedures and management techniques for these issues were reported. Furthermore, the guidance for prevention of various surgical complications was proposed.
