摘要

對於同時合併有上顎牙齦過度裸露、牙齦線不規則及牙冠長度不足導致固定牙橋容易脫落，因牙周生物寬度受到侵犯而造成牙齦發炎的問題，牙冠增長術是一種治療方式；然而，前牙美觀區的處理比較複雜，臨床操作耗時且未必能得到良好的效果。本篇報告是利用軟性透明牙托製作成上顎前牙永久膺復及自然牙牙冠邊緣之手術定位模型，完成簡易明確的前牙美觀區之牙冠增長手術治療，以縮短手術時間，減輕病人術中不適，並且獲得理想的效果。

The clinical crown lengthening procedure is a common technique to obtain proper crown-root ratio. However, for the anterior esthetic region, gummy smile, asymmetrical gingival line, short crown etc. would complicate to the surgery and the result. The purpose of this paper is a case report by using a vacuum former to duplicate the gingival margin of permanent restorations as a surgical stent for crown lengthening procedure, we can get the predictable and precise gingival line as the result in an easy way. The periodontal, photography and radiography examinations were recorded and all in stable condition after 1 month and 3 month postoperatively.
