The clinical crown lengthening procedure is a common technique to obtain proper crown-root ratio. However, for the anterior esthetic region, gummy smile, asymmetrical gingival line, short crown etc. would complicate to the surgery and the result. The purpose of this paper is a case report by using a vacuum former to duplicate the gingival margin of permanent restorations as a surgical stent for crown lengthening procedure, we can get the predictable and precise gingival line as the result in an easy way. The periodontal, photography and radiography examinations were recorded and all in stable condition after 1 month and 3 month postoperatively.
