背景 植牙在口腔復健的需求與日俱增,側方開窗的上顎竇增高術是上顎後牙區常用的術式。本篇臨床報告是2006到2009在奇美醫學中心牙科部利用側方開窗的上顎竇增高術解決上顎後牙區骨量不足的系列病例報告。 材料與方法 從2006年六月到2009年八月共有63位患者(男性31位,女性32位)接受側方開窗的上顎竇增高術。平均年齡47.21±9.02歲。側方開窗的上顎竇增高術總共71個包括31個在右側,40個在左側。收集資料包括系統狀況、吸煙史、植體數目、側方開窗上顎竇增高術同時植牙或階段性植牙、手術使用的材料及手術併發症。所有資料經由SPSS(上標 ®)統計軟體分析。 結果 54息者沒有明顯的系統性疾病,大多數都沒有吸煙史,只有兩位有吸煙。39位施行階段性植牙,32位於側方開窗上顎竇增高術同時植牙。本報告中總共植入71根人工植牙,其中兩根因手術併發症被拔除。大部分的側方開窗上顎竇增高術均放入骨粉及洞。覆蓋可吸收再生膜。手術併發症包括一例Schneiderian membrane連續兩次破洞,一例術後發生鼻竇炎及一例術後傷口感染。 結論 本臨床資料顯示利用側方開窗的上顎竇增高術解決上顎後牙區骨量不足的術式是可預期的。不過手術併發症依然存在的,它的發生率在本系列病例報告中是4.22%。
Background There are increasing needs of dental implant in oral rehabilitation. Lateral window technique of sinus elevation is prerequisite to augment maxillary posterior area with minimal residual bone height. The purpose of this clinical report focuses on clinical data of sinus elevation using lateral window technique in the Dental Department of Chi Mei Medical Center from 2006 to 2009. Materials and Methods There were 63 patients (31 males and 32 females) who received sinus elevation using lateral window technique during June 2006 to August 2009. Average age of the population was 47.21±9.02 years. 71 sinus elevation procedures using window technique had been carried out including 31 sinuses on right side and 40 sinuses on left side. Data were collected from the following parameters: systemic condition, smoking history, implant numbers after sinus augmentation, staged or simultaneous implant placement during the procedure, surgical procedure including membrane and bone graft, complications of the procedure. All data were processed and analyzed using SPSS(superscript ®) statistic software. Results There were 54 patients who were general healthy without obvious systemic disease. Most patients were nonsmokers and only two patients were smokers. 39 patients who received dental implants were staged approach and 32 patients were simultaneous implant placement during sinus elevation procedure. Until now 71 implants were placed and two implants were removed due to sinusitis. The open window of most cases was filled with bone grafts to fill the space and was covered with resorbable membrane. Complications included one case with two consecutive Schneiderian membrane perforation, one case with postoperative sinusitis, and one case with postoperative infection. Conclusion This clinical data which indicates that the result of sinus augmentation with lateral window technique is highly predictable. However, postoperative complication still exists and the incidence rate is 4.22%.
