In order to achieve optimal implant treatment outcomes, a thorough examination, a comprehensive treatment plan and ideal implant positioning are essential. This paper aims to provide a guideline to ideal implant positioning from the perspectives of soft and hard tissue manipulation as well as timing. The consequences of violating any of six items, namely bucco-lingual, mesio-distal, apico-coronal, overlying soft tissue thickness, implant angulation and timing of implant placement, are thoroughly reviewed and discussed. Guidelines describing how to respect these factors during implant planning and placement are developed and presented. In conclusion, placing an implant in an ideal position needs to consider all of the above discussed factors and such an approach is the most effective way of achieving long-term implant success.
