The Prevalence and the Association with Periodontal Health in Teeth with Palato-Radicular Groove: Clinical Examination and Computed Tomography Image Analysis

顎側牙根發育溝(palato-radicular groove)為一發生在上顎門齒的發育變異,本文藉由口內臨床檢查及電腦斷層影像分析探討上顎門齒顎側牙根發育溝的發生率、分類、與牙周健康之相關性。研究結果中,43位患者共157顆上顎門齒中,有46顆有牙根發育溝,其發生率為29%,且發生在上顎側門齒的比例明顯高於上顎正中門齒。依牙根發育溝發生位置分類,大多數發生在上顎門齒正中顎側面(78%)。對牙周組織影響上,有牙根發育溝的牙齒其出現≥5 mm牙周囊袋的比例高於沒有牙根發育溝的牙齒。以電腦斷層掃描判別牙根發育溝有無與臨床檢查結果一致性為87%。總結來說,有顎側牙根發育溝的牙齒與較差的牙周組織健康相關,可視為一危險因子,受限於電腦斷層影像解析度,目前無法精準判別牙根發育溝延伸深度,期望未來能夠克服,更能瞭解牙根發育溝延伸深度與牙周健康相關性。

Palato-radicular grooves are developmental anomalies of the maxillary incisors. The aim of the present article is to evaluate the prevalence and classification of palato-radicular grooves, the association with periodontal health in maxillary incisors with palato-radicular grooves through clinical oral examination and computed tomography image analysis. The results showed there were 46 teeth with palato-radicular grooves in 157 maxillary incisors of 43 patients. The prevalence was 29% and the palato-radicular grooves in maxillary lateral incisors were more common than in maxillary central incisors. The highest percentage (78%) of palato-radicular groove was in the mid-palatal side of upper incisors. The proportion of ≥ 5 mm probing depth was higher in the teeth with palate-radicular grooves than without palate-radicular grooves. The consistency between the clinical examination and computed tomography images was 87%. In conclusion, teeth with palato-radicular groove were associated with poor periodontal health and could be a risk factor. Because of poor resolution of computed tomography image, we still could not differentiate the depth of groove extension definitely. We hope to overcome this problem and realize more about groove extension and the association of periodontal health.
