赤蘚醇噴砂於牙周暨植體治療的應用:文獻回顧

Application of Erythritol Powder Air-polishing System in Periodontal and Implant Therapy: Literature Review

摘要


牙周炎屬於一種感染性疾病,其治療以達成感染控制為目標,由於傳統治療工具的限制,噴砂系統(air-polishing system)被應用在牙周炎及植體周圍炎的治療中。其中赤蘚醇(erythritol)噴砂粉末顆粒小、磨耗性低,可有效清創但不過度磨損牙根、填補材料或植體的表面。赤蘚醇作為噴砂粉末,除了機械性地去除牙菌斑,也表現出對牙周致病菌Porphyromonas gingivalis、Bacteroides fragilis、Aggregatibacter actinomycetemcomitans及植體周圍炎相關的Staphylococcus aureus、Candida albicans 等菌種抑制生長的作用。臨床上在牙周治療中使用赤蘚醇粉末噴砂(erythritol powder air-polishing, EPAP)作為輔助性工具,可得到和傳統治療相近的牙周改善。另外,將EPAP應用在植體周圍炎的治療,發炎程度的改善與傳統治療相似,且赤蘚醇噴砂可能對於植體表面有更少的傷害。然而,由於無法去除硬性沈積物如牙結石,赤蘚醇噴砂僅作為常規治療的輔助工具,不能取代包含超音波洗牙和牙齦下結石刮除等機械性清潔,但仍提供另一種控制發炎感染的術式選擇。

Both periodontitis and peri-implantitis are infectious diseases. Their treatment aims at achieving infection control. Due to the limitations of conventional treatment, air-polishing systems are applied in the treatment of periodontitis and peri-implantitis. Erythritol air-polishing powder has small particles and low abrasiveness, which can effectively debride the tooth or implant surface without excessive substance loss of the root, filling material or implant. In addition to mechanically removing dental plaque, erythritol has anti-microbial effect against several periodontal pathogens and microorganism related to peri-implantitis. In periodontal treatment, erythritol powder air-polishing (EPAP) could obtain periodontal improvement similar to which after conventional treatment. Besides, when EPAP is applied to peri-implantitis therapy, the clinical outcome is comparable with conventional instrumentation, while EPAP may be less harmful to the surface of the implant. However, EPAP can only serve as adjunctive therapy due to its inability to remove hard deposits such as dental calculus. Although EPAP cannot replace mechanical debridement including ultrasonic scaling and subgingival curettage, it still provides another treatment option for controlling inflammation and infection.
