被動萌發異常牙齒牙冠增長術美觀和贋復考量：病例報告
Esthetic and Prosthetic Consideration of Crown Lengthening Procedure in Altered Passive Eruption: Case report
摘要
微笑時過度的牙齦暴露通常被大眾和牙醫師認為是不美觀的。周詳的檢查和正確診斷可達到美觀和可預期的效果。根據不同的病因，可以單獨使用牙周手術、贋復物的形態上調整、矯正治療改善，也可以採用跨科方法綜合治療。本篇病例報告包含兩位女性病患，以牙冠增長手術和贋復治療改善因被動萌發異常導致的牙齦過度暴露，病因依Coslet學者分類分別為被動萌發異常IB型和被動萌發異常IA型合併上顎齒槽骨挺出。患者接受牙齦切除術合併骨整形術和骨切除術，並以贋復物重建齒列，手術後12到14個月追蹤，臨床檢查結果兩患者呈現輕鬆且自然的微笑，牙齒露出適量，外形和諧，恢復正常咬合功能和美觀。因此進一步討論被動萌發異常的病因診斷和相對處置，和兩案例臨床牙冠的美觀和贋復物的設計差異。

Excessive gingival display while smiling might be a main concern for both patients and clinicians. Thorough examination followed by right diagnosis is imperative for achieving an esthetic and predictable results. The therapeutic goal can be achieved surgically, prosthodontically, orthodontically, or by multidisciplinary approach. Two patients were enrolled in this study. This case report demonstrate that esthetic and restorative crown lengthening procedure (CLP) may be used for treatment of excessive gingival display caused by type IB altered passive eruption(APE) and type IA APE combined with dentoalveolar extrusion. At the 12 to 14 months postoperative visit, two cases had stable result with pleasing smile and healthy periodontal tissue. The etiologies, diagnostic tools, diagnosis and corresponding treatment were discussed. In addition, the subtle differences of clinical crown design between two cases are highlighted.
