摘要
隨著高齡化社會的到來，成年人以及中老年人口對於生活品質的要求日益提升。口腔健康為影響人類生活品質一部份，而齒顎不正（malocclusion）對於病患的生理功能及心理層面皆有不良影響，因此民眾對於成人矯正的需求也逐年增加，其中「治療時間的長短」是最為影響成年人接受矯正與否的考量。近一個世紀以來，有許多利用牙周手術去影響牙齒移動的術式被提出，各種縮短矯正治療時間的方式也應運而生，其中與牙周病科最為相關的概念，便是骨皮質切開術輔助矯正治療（corticotomy-assisted orthodontic treatment, CAOT），而其中最具代表性的術式，便是結合選擇性齒槽骨皮質骨切術（selective alveolar corticotomy）與植骨（bone）所產生的牙周輔助加速成骨矯正治療（periodontal accelerated osteogenic orthodontics, PAOO）。本文將介紹CAOT的歷史發展，並延伸到PAOO的創新，討論PAOO的手術觀點及其相關的適應症及禁忌症，提供牙周病醫師關於CAOT一個系統性的回顧。

As population ageing is taking place in Taiwan, adult and middle aged population continue to have an increasing demand toward better quality of life. Among these demands, oral health takes an important part that has a crucial influence on the quality of life. For example, malocclusion can have adverse negative effects physiologically and psychologically on patients. Meanwhile, adult patients' need toward orthodontic treatment also increase year by year. "The periods of treatment time" is the most important factor that dictates patients' acceptance toward orthodontic treatment. In the past century, instead of lengthy conventional orthodontic treatment, there have been many surgical procedures that use periodontal surgery to affect tooth movement. There have also been various methods to shorten the orthodontic treatment time. Among them, the concept, most relevant to periodontology, is the corticotomy-assisted orthodontic treatment. Periodontal accelerated osteogenic orthodontics (PAOO) is most representative CAOT procedure that combines selective alveolar corticotomy and particulate bone grafting. This article will introduce the historical development of CAOT and further extend to PAOO innovation. Furthermore, this article will discuss surgical perspective of PAOO including its indications and contraindications, and provide a systematic review of CAOT for periodontists.
