Clinical Outcomes in Bone-Level and Tissue-Level Implants: Literature Review

摘要


以植牙方式重建缺失的牙齒是現代治療的趨勢,單顆或局部多顆植牙的植體存活率在十年的追蹤為95%。而植體周圍骨高度(peri-implant bone level) 被視為影響植體成功的因素之一,也是保留齒間牙齦乳突和牙齦邊緣完整性的重要條件。當植體在二階手術(second-stage surgery)或植牙後立即接出支台(abutment)而暴露在口腔環境後便開始進行植體周圍骨重塑(peri-implant bone remodeling),造成植體周圍的骨脊高度(crestal bone level)的改變。本篇文獻針對骨水平植體(bone-level implant)和軟組織水平植體(tissue-level implant)進行文獻回顧,討論其邊緣骨喪失(marginal bone loss)及美觀(esthetic)表現。

The rehabilitation of partial and total edentulism using dental implants has shown highly satisfactory clinical outcomes. Multiple studies have proved that long-term (≥10 years) implant survival are around 95 %. The peri-implant bone level has been used as one of the criteria to assess the success of dental implants, also an important prerequisite for preserving the integrity of gingival margins and peri-implant papillae. The peri-implant bone remodeling occurs once the implant is exposed to the oral environment in a second-stage exposure surgery or when the abutment is placed immediately after implant placement leading to crestal bone level changes. This article reviews the differences of marginal bone loss and clinical outcomes between bone-level and tissue-level implants.
