再生手術後矯正牙齒移動的時機:文獻回顧

Timing of Orthodontic Tooth Movement after Regenerative Surgery: A Literature Review

摘要


在全球人口高齡化的現在成人矯正治療人口明顯增加,相對於青少年矯正族群來說,成人矯正人口多伴隨有潛藏的口腔疾病例如:牙周病。牙周疾病引發的牙周骨缺損、缺牙後的齒槽脊萎縮、或是因為年紀的增加沒有生長潛力,會導致牙齒移動的邊界受到限制。由引導組織再生手術重建缺損的牙周支持組織或引導骨再生手術改善齒槽骨條件可以改善此一問題。而再生手術後何時為適當的時機開始矯正牙齒移動,可以不影響再生效果並且能在有限的時間內加快牙齒移動速度,減少整體跨科治療的時間。本文針對此一議題相關期刊做文獻回顧,了解過去研究及最新發展,讓臨床醫師對此主題有更深入的瞭解並且能有新的臨床操作指引作為參考。

The demands that make up adult orthodontic treatment have increased as times have changed and the orthodontic treatments appropriate to adult patients differ in many ways to those used routinely on adolescent patients. Adult orthodontic patients usually suffer from more complex dental conditions, including periodontitis and/or missing teeth. The boundary of tooth movement in adult patients is limited by the periodontal bony defects caused by periodontitis, with the presence of narrower alveolar bone being the result of missing teeth and the reduction in growth potential brought about by aging. The regeneration of periodontal supporting tissue by guided tissue regeneration and the regeneration of alveolar bone by guided bone regeneration are both able to help alleviate this problem. The correct timing of orthodontic tooth movement after regenerative surgery is able to facilitate tooth movement without jeopardizing any regenerative effect and also helps to reduce total treatment time when carrying out this interdisciplinary dental treatment. This review article integrates published evidence on this issue and tries to create some guide lines that can be used in clinical practice.
