摘要


磁振導引高能聚焦超音波(簡稱神波刀)在肌肉骨骼系統中,主要應用於癌症骨轉移、小面關節炎等疼痛緩解以及局部腫瘤控制。對於骨轉移,神波刀可有效安全控制疼痛,比放射治療更快速達到治療效果,且無輻射劑量限制。在原發性惡性骨腫瘤方面,神波刀作為救援性治療,可達到完全或部分消融的效果,或是與化學治療結合,提高病人存活率。對於小面關節炎,神波刀相對於其他治療方式,具有無創的優勢。然而,神波刀也有一些限制,如磁振造影禁忌症、病灶位置限制、治療過程較長。總體而言,神波刀為一種安全、有效且快速的非侵入性治療方法,提供病人另外一種選擇,未來更多的研究可能會使其成為相關疾病的首選治療方式,或是增加更多的適應症。

Magnetic resonance-guided focused ultrasound (MRgFUS) is primarily applied in the musculoskeletal system for pain relief, particularly in conditions such as bone metastasis and facet joint arthritis, and bone malignancy control. For bone metastasis, MRgFUS can effectively and safely relieve pain, achieving therapeutic results more rapidly than radiation therapy and without dose limitations. In the case of primary malignant bone tumors, MRgFUS, as a salvage therapy, can achieve complete or partial ablation effects, either independently or in combination with chemotherapy, thereby enhancing patient survival rates. In the context of facet joint arthritis, MRgFUS possesses advantages over other treatment methods due to its non-invasive nature. However, MRgFUS also has some limitations, such as contraindications for magnetic resonance imaging, restrictions on lesion locations, and a relatively long procedure time. Overall, MRgFUS is a safe, effective, and rapid non-invasive treatment method, offering patients an alternative choice. Further research in the future may establish it as the preferred treatment for relevant diseases or expand its range of applications.
