“臨床溝通演練”作爲醫學能識課程之檢討

The Evaluation of Clinical Communication Drills as General Education in Medicine

摘要


對人生與人性的關懷是一個醫病之間不可或缺的要素,而在醫學敎育裹,「視病猶親」不應是理念與口號,更需要從心靈的體會與誠摯的領悟去落實。成大醫學院的全人教育理念是「作一個良醫之前要先成功作人」,敎導醫師應該在祛病之外,更要醫心。為達此目地,將人文通識課程排在尚未開始接受任何醫學知識敎育的一、二年級,以期突顯其不可或缺的重要性,並達到所謂零歲敎育的效果。「臨床溝通演練」,只是一連串人文與通識課程中的其中一個,重點是讓醫學生提早體認溝通在醫病關係的重要性,並在心態與能力上都有所提升。藉著對其中一個年級的意見調查,只是對此課程與精神的檢討依據。結果發現,醫學生對課程了解及欣賞的程度與老師的用心指導與否與學生的個人努力程度皆密切相關。而學生在訪談病人與家屬的課題心得上產生很大的差異,是值得分析之處。所希望的是有相同理念的醫學敎育工作者,能夠不斷分享經驗,互通有無,以期可以在台灣每個醫學院都達到理想全人醫學敎育的境界。

Insight into the value of human life andconcerns about human suffering are basic qualitiesrequired of physicians to develop sounddoctor-patient relationships.Treating diseaseswithout compassion is curing without healing.Empathy and understanding by physicians ofthe physical and mental distress of patients areemphasized frequently enough in the medicaleducation.Yet efforts are lacking to developeffective methods to ensure that the need forthis sort of attitude becomes more than merelycurricular rhetoric and can be translated intoactions by students.It is critically important,therefore,that courses be developed to trainand indoctrinate students so that these qualitiesbecome an integral part of their medicalcareers.In order to achieve this goal,we believethat a course should be given in the firstor second year,when students' professionalviews and behavior are still in the formativestage,and that it should include drills in clinicalcommunication.The overall goal of thecourse would be to prepare students to face patientsexperiencing distress and suffering and toequip them with the skills to effectively handlethese situations to the overall benefit of thewelfare of patients.This report evaluates such acourse based on an analysis of a questionnairesurvey conducted among second-year medicalstudents upon conclusion of the course.The resultsrevealed that the appreciation of thiscourse depended on both the efforts of teachersand those of students.The best example ofabove statement was the great variety betweenthe responses of students in the interview withthe patients and their families.We believe thatthese results will be of value to teachers whoare designing similar courses to emphasize caringand empathy in the practice of medicine.(Full Text in Chinese)
