摘要


臨床上對於合併牙齒矯正和正顎手術後治療的病例,矯正醫師可以以病人治療前的測顱影像為模板,來進行病人手術移動硬組織和軟組織變化的預測描繪,這樣的預測方式稱為Visual treatmentobjectives (VTO),可分為兩種:一是先改正牙齒角度,再將顎骨位移至理想位置;或是以達成術後較美觀的軟組織外觀為主,再去回推顎骨位移以及術前牙齒如何移動,來獲得術後合理且理想的咬合及外觀變化。本篇文章將回顧其原則與相關文獻,並以病例報告來比較這兩種VTO之不同。

In the cases requiring combined orthodontic and orthognathic surgical treatment, orthodontists can predict post-surgical hard tissue and soft tissue changes based on rules regarding facial esthetics and functional occlusion. There are two different Visual treatment objectives (VTO) to predict the surgical outcome for the lateral profile with the initial lateral cephalometric tracing as a template. One is skeletal-based VTO, which corrects the inclination of teeth first, then resolves the skeletal discrepancy by surgery. Another is soft tissue VTO, which makes the prediction of jaw displacement first to achieve esthetic and harmonious post-surgical profile, then plans the movement of teeth to obtain ideal overbite and overjet. Principles regarding these two methods of VTO are described, and one case is used as an example to illustrate the possible differences between these two predictions.
