摘要

本篇為一下顎前突合併下顎歪斜之骨性三級患者，治療上以手術優先之正顎手術矯正治療取代傳統先進行術前矯正去代償的觀念與作法。從結果來看，這種治療方式的好處為：在治療一開始即可達到患者外觀的改善，同時在本案例也有縮短整體治療時程的效果。關於手術優先之正顎手術矯正治療的作法，其運用的概念、特殊的設計考量、長期穩定性、以及臨床應用等也會在本篇文章中作廣泛的討論。

A case of mandibular prognathism with facial asymmetry was presented. Surgery-first orthognathic approach was performed that no pre-surgical orthodontic treatment was done before surgery. Within this treatment modality, patient had major profile improvement in the beginning of the treatment and total treatment time was shortened. The philosophy, clinical application, long term stability, and special consideration of the surgery-first orthognathic approach were discussed.
