活動式慢速腭擴張器在上顎橫向發育不足之成人患者的運用－病例報告
The Application of Removable Ball-type Palatal Expander in Adult Patients with Maxillary Transverse Deficiency-Case Report
摘要
臨床上，上顎牙弓橫向發育不足的病人，常見後牙區錯咬及牙弓窄小的情形；這對發音、咀嚼功能甚至美觀方面有著一定的影響。在兒童時期， 可藉由快速腭擴張器的矯正裝置，來刺激上顎骨縫合區的生長發育，改善上述等問題。然而成人的上顎縫合區已經閉合，若要做大幅度的上顎骨擴張，往往必須藉由手術的方式來輔助解決。本報告提出兩例這類成人病例，利用配戴活動式球型腭擴張器並慢速調整此裝置，根據骨重塑的原理，來引導上顎牙弓做有限度的橫向擴張。經過數個月的治療後，有效的改善上顎牙弓窄小及解決後牙區錯咬等問題。（J Taiwan Assoc. Orthod. 24（1）: 24-34. 2012）

In clinical, the maxillary transverse development insufficiency of the patients often have the posterior cross-bite and arch narrow circumstances, there are some impact in the pronunciation, chewing functions, and even for appearance. In childhood, these problems can be improved by rapid palatal expander correction device to stimulate the maxilla suture zone growth and development. However, the adult's palate suture zones have been closed; if the maxilla needs to do a significant expansion, it often has to assist by the way of surgery to resolve. This report presents two examples of these adult cases utilize wearing the removable ball-type palatal expander and the slow adjustment of the device, according to the principle of bone remodeling, to guide the maxilla for limited lateral expansion. After several months of treatment, the narrow upper dental arch, posterior cross-bite and other issues are improved effectively. (J. Taiwan Assoc. Orthod. 24(1): 24-34. 2012)
