為推動醫學教育對臨床推理(clinical reasoning),本研究目的在分析初始2018-2020年間各大醫學院醫學系舉辦此創新臨床測驗競賽逐年之情況,報告各樣影響測驗品質因素之執行方式之演變,與執行成效的呈現,以建立「虛擬實作型臨床技能測驗」的舉辦成功要訣。研究方法:以回溯性研究設計蒐集2018-2020年間,三屆全國臨床技能競賽的逐年各樣建立與執行之相關資料,包括:案例發展、資訊/軟硬體等考場設備、及測驗行政作業,加入考生回饋及測驗運作順暢度。以質性分析描述該測驗在導入期如何進入穩定運作之過程。測驗工具為醫諾華「虛擬臨床診療訓練系統V-DxM」,提供臨床技能測驗所需之「虛擬病人」、「虛擬醫院」、「考官」以及測驗過程紀錄。結果:三年總參加人數369人,考生為醫學生與專科護理師,合作進行測驗的機構含資策會與5個醫學院醫學系,在台灣北中南三考場同步進行。案例發展、資訊/軟硬體等考場設備、及測驗行政作業逐年改進。面對大量考生,虛擬臨床技能競賽歷經導入階段中面對眾多挑戰,經過逐年改善而成為可行,比起傳統實體OSCE臨床實作測驗具眾多優勢。此虛擬競賽提供了未來高標準虛擬OSCE施作的導入經驗。
For the purpose of advance medical education in clinical reasoning, this study focus on analyzing the annual progression of the innovative clinical examination competition held by various medical schools from 2018 to 2020. The report focuses on the evolution of factors affecting test quality and the implementation effectiveness, and hence establish the keys for conducting "virtual practical clinical skills assessments." The Research Methods includes collecting relevant data from the nationwide clinical skills competitions held between 2018 and 2020. These data cover case development, exam venue equipment such as information, software, hardware, and administrative operations. Feedback from participants and stakes holders were also considered to examine the smoothness of the procedures. The assessment tool is named the Innova Med Tech V-DxM, which provide "virtual patients," "virtual hospitals," "examiners," and test process were recorded for clinical skills assessments. Results: A total number of 369 participants were involved in the three years period, including medical students and nursing practitioners. The collaborating institutions included the Institute for Information Industry and five medical schools, with the competitions held simultaneously at three locations in northern, central, and southern part of Taiwan. Annual improvements were achieved in the case development, exam venue equipment, and administrative operations. The virtual clinical skills competition faced the challenges of large-scale candidates during the introduction phase, and has now become more feasible through yearly improvements. The virtual simulation examination offers many advantages over traditional physical OSCE clinical skills tests. This virtual competition provided valuable inputs for implementing high-standard virtual OSCEs in the future.
