糖尿病患下肢傷口癒合不良之病例報告

A Case Report Involving Poor Healing of Lower Extremity Wounds in a Diabetic Patients

本案為一位四十三歲男性患者,有高血壓及糖尿病史,規律服藥但數值控制不佳,在2019年4月22日發現右小腿脛骨內側紅腫、局部疼痛且高燒至39°C,求治風濕免疫科診斷為蜂窩性組織炎,服用Augmentin一週後紅腫範圍仍逐漸擴大,並伴隨間歇性發燒,故於2019年4月29日收置住院治療。患者就診中醫已間隔兩個月,而傷口遲遲未能癒合,呈現紅腫熱及按壓疼痛,且周圍皮膚轉為紫暗色,經濃縮中藥粉劑及針灸治療後,瘡口紅腫消退,疼痛減緩並收口癒合。

A 43-year-old male patient has a history of hypertension and diabetes. Despite regular medication, his medical checkup indicators deviated from norm.. On 2019/04/22, his medial shin at the right calf presented red and swollen and he had local pain and a fever of 39 °C. A rheumatism immunity department diagnosed cellulitis. He took Augmentin for a week, yet the range of redness and swelling continued to expand gradually, accompanied by intermittent fever. He was accordingly hospitalized on 2019/04/29. It had been two months since the patient sought help from a traditional Chinese medicine (TCM) doctor, with an unhealed wound, redness, swelling, hot and pressing pain and dark purple surrounding skin. After treatment with a concentrated powder of Chinese medicine regime and acupuncture, the redness and swelling subsided, pain was relieved and the wound healed.
