桃園地區各醫院間急診策略聯盟試辦模式

摘要


配合行政院衛生署「改善醫院急診重症醫療計劃」之政策,「桃園區域醫療網內醫院整合(建教)試辦計劃」由桃園地區醫學中心、區域及地區教學醫院各一所實施於民國八十七年七月一日至八十八年六月三十日,本論文主要在探討此三所醫院急診部門進行整合時,其溝通協調與制度規劃之過程及執行之狀況。結果發現除他科醫師在照會原則及是否前來診視病患有相當大的歧見外,在整合時相關急診醫護人力、企業識別體系、急診駐診制度、雙向交流制度、醫療風險分擔制度及轉院制度均能達成共識。本論文指出主事者的意志與管理,參與者對制度的認同,均能影響醫院急診部門策略聯盟的成功與否。

A strategic Emergency Department alliance between a medical center, a regional hospital and a community hospital in Taoyuan county ren from July 1, 1998 to June 30, 1999, and was supported by the Department of Health. This paper focuses on the process of formatting, communicating and implementing the plan while the three emergency departments developed the alliance. The plan was executed smoothly in terms of medical staffing, corporate identity systems, duty rules, education, risk sharing, and patient transfer rules. Problems in consultation and manpower support across departments persisted despite discussion and trials. This report describes how the willingness and management abilities of those in charge, as well as identification of key participants strongly influenced the success of the emergency department strategic alliance.
